If your child will need to have prescription medication administered at school for any reason, the parent/guardian must complete pages 1 and 2 of this consent form and the child's prescriber/physician must complete page 3 of this consent form. Please bring the completed forms and your child's medication in its original prescription bottle/packaging to the school nurse on the first day of school. Students may NEVER self-transport medication to or from school.

This letter is for parents of grade 7 students regarding the state-mandated SBIRT Screening that will take place for seventh graders only. 

This form is for parents of grade 7 students ONLY who wish to opt out of the SBIRT screening process.
